An exceptional opportunity is available for a BC/BE Family Medicine or Internal Medicine physician to take over an established membership-based primary care practice in Manchester Center, Vermont. This unique opportunity allows a physician to step into an existing patient panel and continue providing highly personalized care.
This well-established practice delivers concierge-style primary care with a strong emphasis on prevention, wellness, healthy aging, and comprehensive chronic disease management. Services include annual health assessments, advanced screenings, preventive cardiology, and a collaborative physician-patient partnership approach tailored to each individual's long-term health needs.

Position Highlights:
Established member-based patient panel with immediate practice continuity
Lower-volume setting designed to support personalized patient interactions

Regional patient base serving communities throughout Vermont, western Massachusetts, and eastern New York
Hospital affiliation with Southwestern Vermont Medical Center and established referral relationships with Rutland Regional Medical Center and Dartmouth Hitchcock Medical Center

Qualifications:
Board-Certified or Board-Eligible in Family Medicine or Internal Medicine
Currently licensed in Vermont or eligible to obtain Vermont licensure

Experience delivering comprehensive longitudinal primary care and chronic disease management
Interest in preventive cardiology, nutrition, weight management, and healthy aging initiatives
Strong interpersonal and communication skills

Desire to become a trusted healthcare leader within the community
Annual Compensation: $350,000 to $500,000 per year
