Company Overview
Pueblo Community Health Center (PCHC) offers a Patient-Centered Medical Home for all who seek care.
Our practice model emphasizes a personal relationship with a Primary Care Provider (PCP).
PCPs lead and coordinate services with the help of a team of professionals. Our goal is simple: better care for you. We place importance on comprehensive, continuous attention to you, for your improved health outcomes.
The system has 12 sites which, together, provide medical, dental and behavioral health and two on-site pharmacies and labs. The main clinic is located in a beautiful, three-story building which operates with the latest technology. The newest site, East Side Clinic, opened in January 2022, and is The First Net-Zero Verified Medical Facility in North America.
Company History
Early Foundations
1983 – PCHC began providing primary health care services to Pueblo’s underserved population, originally located on the grounds of the Colorado State Hospital.
1986 – Relocated to St. Mary-Corwin Hospital.
1987 – Purchased Routt Avenue Clinic, which remains the administrative offices today.
Growth in Facilities & Services
2000 – Acquired and renovated the Lake Avenue Clinic to expand dental services.
Satellite clinic opened in Avondale (rural community).
Grand Avenue – Clinic established for homeless patients.
Expansion into Underserved Communities
2002 – Opened Park Hill Clinic serving Pueblo’s most underserved neighborhood.
2005 – Opened a patient pharmacy in the lower level of Park Hill Clinic.
Major Modernization
2009 – Opened a new 41,000 sq. ft. state-of-the-art clinic at 300 Colorado Avenue after a large capital campaign.
2014 – Expanded that site by 7,000 sq. ft., adding capacity for 10,000 additional patients over the next decade.
Latest Development: East Side Clinic
October 5, 2020 – Broke ground on a 64,000 sq. ft. integrated health care facility, located in a neighborhood with a high concentration of Medicaid members.
2022 – East Side Clinic officially opened, offering team-based integrated medical, behavioral, dental, pharmacy, lab, and case management services.
Goal: Serve 5,000 additional patients over the next 10 years.
