Job Title: Primary Care Physician (MD/DO)
Location: Phoenix, AZ
Schedule: Full Time (Mon-Fri 0800-1700)
Patient Load: 15-25 Patient/Day Average
Wage: $200,000 - $250,000 / year base (plus bonus)
Benefits: Full Benefit Package
Call: No Call
Setting: Outpatient Family Medicine / Primary Care Clinic

Welcoming Family Medicine / Internal medicine / Primary Care Physician to join our team. We are open to
experienced providers or new graduates. Please review all duties before applying and include a CV and any

Responsibilities

• Well visits: Well Woman Exams, Complete Annual Physicals, AWV
• Same day appointments (fever, UTI, headache, allergies, etc)
• Chronic disease management (diabetes, hypertension, COPD, etc.)
• Preventive care, healthy living, disease prevention
• Ordering Diagnostic Testing & Labs as Needed
• Ability to Understand and Work with Specified Care Flows of Treatment
• Ability to use EHR System Proficiently & Complete Notes in a Timely Manner

Requirements :

• Licensed Medical Provider (MD/DO)
• DEA License Without Restrictions
• No History of Malpractice
• No restrictions with Credentialing with Medicare/medicaid/Commercial Insurances
