Our multidisciplinary team includes primary care providers, complex care managers, nurses, medical assistants, behavioral health clinicians, and community health workers, supported by onsite infusion services, pharmacy, lab, and imaging suites. We also address social determinants of health such as housing instability, food insecurity, and social isolation.
Role Overview
The Lead Physician provides direct clinical care to a panel of members and serves as a clinical leader, guiding best practices in value-based care. Working closely with the Medical Director, this role mentors providers and care teams, drives quality and utilization goals, and represents AbsoluteCare in the community.
Key Responsibilities
Deliver excellent primary care to a panel of members and lead your interdisciplinary care team.
Coach and mentor providers on delivering high-quality primary, urgent, and community care.
Provide clinical guidance to nurses, MAs, and fellow providers on complex cases.
Partner with the Medical Director to achieve quality, utilization, and engagement goals.
Serve as an ambassador to community organizations, hospital systems, and payer partners.
Participate in leadership meetings and contribute to strategic initiatives.
Duties Include
Provide primary care and urgent care services.
Accept same-day/next-day appointments for post-hospital or ER discharges.
Practice evidence-based medicine aligned with the PCMH model of care.
Track and achieve specified quality metrics.
Develop transitional care plans and manage patients across the continuum of care.
Utilize technology and EHR systems to optimize quality and efficiency.
Participate in multidisciplinary meetings and develop clinical protocols.
Approximately 70–80% of time dedicated to direct clinical care.
Qualifications
Board-certified physician (Family Medicine or Internal Medicine preferred).
Experience in value-based care and managing medically complex populations.
Strong leadership and mentoring skills.
Commitment to team-based care and addressing social determinants of health.

Requirements
Minimum Qualifications
- Active, unrestricted medical license in the state of employment.
- 5+ years of clinical experience and administrative experience leading interdisciplinary teams.
- Experience working for a clinic, community-based organization & inpatient unit.
- Experience with training and mentoring.
- Excellent interpersonal communication skills.
- Strong problem solving, project management and organization skills.
- Independent self-starter and critical thinker interested in population health, healthcare utilization management, and the ongoing development of our care model.
Working conditions
This job operates in a professional office environment. This role routinely uses general office equipment. Often wear a special uniform, such as a lab coat or surgical scrubs, protective or safety attire, such as gloves and masks. Exposed to the diseases and infections of patients daily. May share workspace with other technicians and staff. Will be required to travel to any of our centers or corporate office based upon company needs.
