Were seeking a passionate and driven Family Medicine Physician Assistant or Nurse Practitioner to join our primary care team in Shelby, MI! This is an exciting opportunity to make a meaningful impact on the health of our community while working in a supportive, mission-driven environment.

Opportunity Details

• Outpatient Department of Trinity Health Shelby Hospital
• Our Shelby locations offer primary care services with a team of Providers that specialize in Family Medicine working together to deliver a comprehensive, personalized patient experience
• In addition to primary care services, our Shelby office offers specialty services to including general surgery consultations, Urology, Sleep Medicine and Urgent Care
• Practice currently has a team of 6 total providers with 13 rooms
• 4 days/week schedule with extended hours and no weekends
• No on-call requirement for APP
• Very close knit & collaborative office with a supportive environment

Requirements

• Open to strong new graduates 
• APPS: Board-certified in specialty prior to employment
• Active Michigan medical license (or ability to obtain)
• DEA license
• BLS certification (must be maintained)/ ACLS/BLS certification (must be maintained)

Duties & Responsibilities

A typical day may include:

• Providing direct patient care
• Collaborating with multidisciplinary teams
• Participating in teaching and mentoring
• Documenting in Epic EMR
• Engaging in quality improvement initiatives
• Supporting community health efforts
